Predicting difficulty in laparoscopic cholecystectomy preoperatively using modified Randhawa scoring system

Background: Laparoscopic cholecystectomy is a low-risk, minimally invasive operation and currently the standard for treatment of gallstone gallbladder disease. Preoperative assessment difficulty required to prevent problems, ensure readiness, an effective course surgery. Methods: In our work, we attempted use modified grading system developed by Randhawa et al. preoperatively predict difficult laparoscopic cholecystectomy. Age, gender, illness duration, history prior GB disease, BMI, abdominal scar location (infra or supraumbilical), palpable gallbladder, sonographic findings (gall bladder wall thickness, pericholecystic collection, impacted stone) were assessed in patients who had been diagnosed with stones LC. Depending on individual surgeon's assessment, procedure rated as easy, difficult, very difficult. Results: On comparison test group level predicted gold intra op has sensitivity 51.9 % specificity 100%. The positive predictive value 100% Negative 91.5%. agree 154 out 167 having diagnostic accuracy 92.21%. Kappa 0.644 indicates good agreement p <0.001. Area under curve 96.1% Total score significant <0.001 Conclusions: current Pujahari scoring method useful appropriate forecasting operative outcome
